Directions:

As you may recall, we must match each dollar of federal funds we receive for this grant. Thus to "go for" the grants maximum of $175,000 we have to identify a like amount of "in-kind" resources/contributions. This is the form on which that "match" is noted. In addition, this is the form on which each participating Deaf Education Teacher Preparation program notes specifically how they will be involved in the grant. I have provided you with the following directions to help you complete this critical form:

1. Partner #: leave this line blank

2. Signature: the Director of your Deaf Ed. Teacher Preparation Program

3. Partner: the institution in which your Program is located

4. Dept/Faculty the names your Program faculty that will participate in the grant

5. Address mailing address for the Director of your Program

6. City… (same as above)

7. Telephone… (same as above)

8. Type of Inst. indicate if your institution is a public or private college or university

Cost Share……

The one key thing to keep in mind here is that you can not use any federal funds to provide the "cost share" for your participation in the grant. All other funds can be used.

1. Salaries… faculty, student and/or staff time that can be committed to support grant activities (e.g., "Graduate Faculty" release time, graduate assistant allocation, 1/4 time of a departmental secretary, etc.)

2. Benefits usually a % (e.g., 28-30%) of the "Salaries" money

3. Travel funds allocated by your institution to support travel

4. Equipment… any new technologies (e.g., faculty computers) that are purchased after the grant has been funded that you will use to support/carryout grant related activities

5. Materials… value of materials and supplies you will use from your institution to support/carryout grant related activities

6. Consultants… not sure myself regarding this one, any suggestions?

7. Other… e.g., the cost of teleconferences, video conferencing, …that your institution will provide to support/carryout grant related activities

The final "chunk" of information that is needed on this form concerns specifically how you/your Program will participate in and support the grants activities (see the rectangular "box" at the bottom of the form). The information in this box is tied to the identified activities for each of the grant's major activities (i.e., "Faculty Development", "Syllabi Development", "Resource Development" and "Support Development." The initial array of those activities can be found under "the role and responsibilities of project…" in the "Grant Rough" (www.educ.kent.edu/deafed/990416b.htm) that I posted a few days ago. I would suggest that you work with the "Lead" individual for the grant's various components to determine what you might want to write in this final section.
